MATCH PENALTY REPORTING POLICY
USA HOCKEY ONLY - ALL OTHERS SEND REPORTS TO LIHO WEBSITE
All Match Penalties must be reported to the local Administrative Supervisor within 48 hours. (Your local Administrative Supervisor is Rich Walker and his phone number is (516) 384-4348.) When reporting, the official must request a NYSAHA Incident Report Form which must be completed and returned (postmarked), along with a copy of the scoresheet, within 24 hours of receipt. This form is also now available on-line in the 2003-2004 Seminars area on the New York District Hockey Officials website www.newyorkhockeyofficials.com and a direct link to the form is http://www.newyorkhockeyofficials.com/NY_MatchPenaltyForm.pdf . Printing this form and sending it to Rich Walker within 24 hours of the incident is the fastest and preferred way of reporting.
URGENT
When a Match Penalty is assessed for an alleged PHYSICAL ASSAULT OF AN OFFICIAL (RULE 601 (G) 1 OR 601 (J) 1), you MUST CONTACT Mike Shapey IMMEDIATELY (in addition to reporting it to the local Supervisor as stated above).
